A familiar logo or saved account name can hide a wrong filer identity in a reused transmittal. This checklist helps B-series preparers verify the entity and contact information on the official 2025 Form 1094-B. It is a source-evidence review, not an IRS identity-matching service or a determination of reporting responsibility from a company name.

Identify the reporting party before checking its details

Confirm the reviewed coverage arrangement and reporting filer. The 2025 instructions distinguish insurers, self-insured sponsors and government coverage providers. An administrator or transmitter may perform services without becoming the reporting provider merely because its name appears on a portal.

Compare the filer determination with the provider context in the associated 1095-B preparation records. If the entities differ, obtain the reporting reviewer's explanation before copying one into the other. An employer listed in a statement's Part II is not automatically the same role as its coverage provider in Part III.

Record the entity's internal key and the source used to establish the reporting role. This makes the review more reliable when several affiliated organizations or service companies share similar names.

Verify name and EIN together

A format check can establish that a field contains nine digits, but it does not prove that the identifier belongs to the intended filer. Compare the name and EIN together against the approved source. Watch for copied values from another entity in the same administrative account.

Preserve identifiers as identifiers in spreadsheets and exports. A numeric conversion can alter display or remove a leading zero. Check the actual saved value and the generated draft instead of assuming the visible cell formatting proves the underlying value is intact.

When an entity's name or organization structure changes, keep the historical reporting context separate from the current master record. Do not rewrite an already reported prior-year identity during routine preparation without establishing whether a reporting correction or another reviewed action is needed.

Verify the contact and correspondence address operationally

Lines 3 and 4 identify the person responsible for answering questions about the B-series filing or information reported. Confirm that the contact can obtain source records and coordinate with the preparer. A generic switchboard can be part of the routing, but the internal team should know how a reporting question reaches the responsible person.

Lines 5 through 8 identify the filer's complete address for correspondence under the instructions. Confirm the approved location and any mail-handling dependency. An office move, remote-work change or provider transition can make a copied address stale even if the entity name remains correct.

Record substitute coverage and the internal correspondence owner in the review packet. Those operational notes support continuity; they do not require inventing new fields on the form or substituting a service provider's information without a reporting basis.

Worked example: a vendor account label is not the filer name

Fictional example: Birch Quay Coverage uses a preparation service whose dashboard account is labelled Birch Quay Administration. A preparer copies that display label into the transmittal and carries forward the former reporting contact's telephone number from last year.

The approved entity record identifies Birch Quay Coverage as the filer and pairs it with the controlled EIN source. The reviewer replaces the account label with the complete approved filer name and confirms the new reporting contact. The correspondence address remains unchanged after verification.

The review resolves two header discrepancies: the filer name and contact number. It does not alter the accompanying return population or silently rewrite prior accepted filings. The corrected current draft is saved as a new version with the source references, so the next reviewer can explain exactly what changed.

Approve the header and maintain the evidence

Compare the generated 1094-B with the approved master record after any mapping or export step. This catches a correct source value that was truncated, transformed or copied into the wrong destination field. Retain a readable draft with its version reference.

Use the review packet to link identity evidence, contact confirmation and unresolved questions. Use the count reconciliation for line 9; verifying the header does not establish that the accompanying inventory is complete.

Assign an owner for later master-data changes and specify when another review is required. A prior-year approved record is useful evidence, but it should not be treated as permanently current. Preserve the source and version actually used for each reporting packet.

Common questions

Can we use the name shown in the vendor's account menu?

Only if it matches the approved complete filer name for the reporting context. A dashboard label may be an administrative or brand name. Verify the source rather than copying it automatically.

Does a nine-digit EIN field prove the filer is correct?

No. It establishes a format characteristic, not assignment to the intended entity. Compare the name and EIN together against controlled evidence and inspect the actual saved value.

Is the employer on a 1095-B always the 1094-B filer?

Do not assume that. Employer context, coverage-provider identity and service-provider roles can differ. Establish the reporting party under the applicable arrangement before preparing the transmittal header.

Why verify an address that worked last year?

The correspondence location or mail-handling responsibility may have changed. Retain the current approved source and identify who receives reporting correspondence rather than relying only on a copied template.

Does this checklist complete the entire transmittal review?
