Form 1094-B identifies the filer and contact, supplies the correspondence address and counts the Forms 1095-B accompanying the transmittal. Review those fields against the actual prepared batch and source records before approving the completed version.

The 1094-B is a short form, but a correct-looking header can still refer to the wrong filer or count the wrong unit. This annotated guide uses the official 2025 form and a fictional three-return packet. It explains the form's meaning; the appropriate electronic or paper filing procedure is a separate determination.

Identify the form year and accompanying packet

Confirm the reporting year printed on the draft and identify the exact Forms 1095-B that will accompany it. Keep an internal packet reference and version so the count review can be repeated. A transmittal copied from another year may carry a familiar name while describing a different batch.

The 2025 instructions describe Form 1094-B as the transmittal used to submit Forms 1095-B. The official 2025 form contains filer, contact, address and total-return fields, followed by its signature declaration.

Use the form as a map to the underlying packet. It does not list every covered individual or establish that the enrollment information inside each statement has been reviewed. Those supporting checks belong in the preparation evidence linked to the draft.

The meanings come from the specific 1094-B instructions and the published form. This legend is a reading aid, not a substitute form or a set of electronic XML field names.

Worked example: three returns cover twelve people

Fictional example: Cedar Inlet Coverage prepares three Forms 1095-B. Return CI-01 includes one covered person, CI-02 includes three, and CI-03 includes eight. The third return uses a continuation sheet for the additional people beyond the main form's rows.

The packet therefore represents 1 + 3 + 8 = 12 covered people, but its line 9 return count is 3. The 2025 instructions say not to count continuation sheets as additional Forms 1095-B. Printing another page does not create another return.

The reviewer compares the final inventory of CI-01, CI-02 and CI-03 with the transmittal, then confirms that the continuation sheet remains associated with CI-03. The example does not determine whether Cedar Inlet may file on paper; electronic filing obligations depend on the applicable rules and broader return context.

Verify who can answer and where correspondence should go

The contact on lines 3 and 4 should be the person responsible for answering questions about the B-series filing or information reported, as the instructions specify. Confirm that the person understands the reporting arrangement and can obtain the underlying records.

Check the telephone number and any operational routing through the organization's established process. A former employee's name or a disconnected number can persist when a prior-year template is reused. Document substitute coverage internally without inventing a second form field for it.

Lines 5 through 8 concern the filer's address for correspondence. Confirm the approved address and source rather than substituting a vendor's address simply because that vendor prepared the packet. Use the issuer information checklist to resolve name, identity and address discrepancies.

Review the final version after any packet change

Give the transmittal and accompanying return inventory matching version references. If a return is added, removed or moved to another packet after the review, repeat the affected count and membership checks. A line 9 value can be numerically plausible while referring to an earlier population.

Inspect each field against its source. Do not treat a successful print preview or format check as proof that the legal identity, contact or return membership is correct. Keep unresolved facts separate from the approved portion of the work.

Use the manager review packet to collect the source evidence and questions. The reviewer should be able to locate a supporting record for the header and a specific inventory behind the count without relying on a preparer's memory.

Complete the review and route the approved information

The signature declaration on the official form concerns examination of the return and accompanying documents. Follow the applicable signing and authorization process for the actual filing method. This fictional guide supplies no real signature, approval or filing result.

The instructions distinguish paper formatting from the schemas and business rules used electronically. A human-readable transmittal draft can support review, but it is not itself proof that an electronic packet has been created or transmitted correctly.

Common questions

Does line 9 count covered people?

No. It counts the Forms 1095-B transmitted with the 1094-B. One 1095-B can contain several covered people, so the people count and return count can differ.

Does a continuation page increase the return count?

No. The 2025 instructions specifically say not to count covered-individual continuation sheets as additional Forms 1095-B.

Whose address belongs on the transmittal?

Use the filer's complete correspondence address under the applicable instructions. Do not substitute a preparer's or vendor's address merely because that organization handled the paperwork.

Does a reviewed PDF prove the electronic filing is ready?

No. A readable draft supports field review. Electronic formatting and submission have separate requirements and outcome evidence. Preserve the approved data and handoff to the authorized filing process.

What should change when another return is added?

Update the final inventory and line 9 count as appropriate, then repeat affected membership and review checks. Keep the approved version associated with the actual packet rather than an earlier draft.
